Speech and Language Daily Lesson Plan and Data Sheet Forms

Several people have complemented my lesson plan and data sheet formats so I thought maybe you can use them too!  After several versions and tweaks, I am happy to share!

This download includes a 3 page Daily lesson plan and a 1 page data Sheet.
The lesson plan has space to write plans for 8 groups per day, plus a "to do" and "notes" section.

Print one data sheet per student and use it for the year to keep track of how your students are progressing.

I find it easiest to write my lesson plans by day (Monday, Tuesday, etc.).  This way, I just grab the lesson plan for the day and I'm ready to go (I copy the first two pages on one sheet so I only use 2 sheets of paper per day).  My lesson plans include the date, time, materials, procedure, student names, target behaviors, and evaluation for each group.  At the end of each session, I transfer the evaluation/ data onto each student's data sheet.

Let It Fall by Maryann Cocca-Leffler Book Companion!

This is a comprehensive speech and language book companion to use with the story "Let it Fall" by Maryann Cocca-Leffler.  There are several activities you can use to target Fall vocabulary and the seasons, without the book too!

The packet includes:
Pages 3-5:  Compare and contrast seasons 
use page 4 as a branching strategy  to have students match vocabulary words to the season(s) they belong with on page 3
Pages 6-8: Sequence the story with the choice of 2 sequencing mats.
Pages 9-12: Target Vocabulary found in the story
Includes a matching worksheet for homework/ extra practice.

Pages 13-14: Concrete “WH” Questions targeting seasons with matching answers..
Pages 15-18: Higher-order Language Questions.  
Use page 15 and print multiple copies as a “mat” for each student.  If they answer the questions correctly, they place the card on the same image.     The first student to fill up their mat wins!
Pages 19-24: Articulation Cards R, S, L (initial, medial and final)
Can use with the same mat on page 15 as a reinforcer.
Or have students find their word in the story for extra articulation and phonemic awareness practice.

Story Caterpillar Web Freebie

Well, I have survived the first month of school! In celebration, I have a freebie to share!

This print and go describing story caterpillar web can be used for any story or passage.  Identify all of the main comprehension targets on one student friendly sheet.
Level 1- characters, setting, beginning, middle, end, title, author
Level 2- characters, setting, main idea, problem, solution, title, author

Picture This! A Describing FREEBIE

I am pretty excited to go back to school and see all my kiddos!  I've been getting organized and working on some materials that I think they will love.

Print. Laminate and cut all cards.
Use page 3 to introduce the topic of describing and offer models.
Let each student choose their own camera to use during the activity for fun.
(page 4)
Students take turns choosing “picture” cards. (pages 5-10).  Go through these cards and select the ones most relevant to your students before use.  There are plenty to choose from and some may not be relevant to every part of the county.
On each students turn, they will read the card silently and describe the place printed using attributes. 
The other students must guess the place being described.
Prompt students to describe the place as if they are describing a picture to someone who has never seen it. 
If another student guesses the place, the student describing keeps the card.
The student with the most cards at the end of the game wins.

Back to School with Anna and Elsa!

Here is a great Back to School packet to use with Pre-K through 2nd grade!  Get to know your new kids this year with a Frozen theme!!

The packet includes the following:
 “Getting to know you” sheets- 4 choices (Anna, Elsa, Sven or Olaf).  Pages 3-6
Emergent reader book.  Page 7


Sight word identification books.  (Target sight words: I, see, look, is, at, and).  Pages 8-10.  Use booklets to identify sight words- have student point to or color target sight word.  Can use for following directions (ex. Every time you see this word “is”, color it red)



Phonemic awareness: Initial and final phoneme identification (laminate and use dry erase markers to cut down on printing!) Pages 11-13.

Following Directions and Prepositions (on, above, below, next to).  Pages 13-15.
Game board to be used with any activity.

Facts versus Opinions in Writing Take 2!! (Freebie until Sunday!)

As the school year is dwindling down, I am trying my BEST to keep the kiddos (and myself) entertained.  They all loved the Reasoning With Rain Facts Versus Opinion activity, so I figured I would make another one!   ( if you missed it, find it here for free)

Here is Petal Pondering.  It's a fun spring/summer themed fact vs. opinion activity complete with a sun themed board and flower sentences.  


It also includes a homework/ carryover sheet for extra practice.
